June 12, 19, 26 & July 10 & 17 from 9:00 a.m. to noon in the courtroom of the Madison Police department on Crawford Street. 

The fee is $80 for the Fall or you can pay for both Fall and Spring for $160. 

The DEADLINE to register is July 17.  All forms postmarked after July 17 must include a $30 late fee and will be placed on a waiting list.  We will refund your money if we are unable to place on a team.  Placement is not guaranteed if you register late.

If you are new to SMCSO or haven't played with us in a few years, you will be required to provide a copy of a birth certificate.

Players must be 4 years old by July 31, 2010 to participate.

It is HIGHLY recommended that you bring your child to registration to size them for the uniform.  SMCSO will not be responsible for incorrect sizes marked on the registration form.  Incorrect sizes could result in delays and extra uniform charges.
